    Proposals must include the following:

    • Primary person and contact information for related correspondence
    • Full professional name, credentials, title, organization, email and CV of EACH person who will attend and present at the CFHA Conference
    • Title of Presentation
    • Presentation Summary (3-5 sentence description of the session)
    • Abstract (detailed description of the session; 1,000 word maximum)
    • Four Learning Objectives (At the conclusion of this presentation, the participant will be able to: identify, list, etc.)
    • Content Level for audience (basic, intermediate, advanced)
    • Preferred session format (15 to 90 minutes, or poster)
    • Suggested session track (education/training, clinical, policy/finance or research)
    • AV and Meeting room requests
    • Terms and Conditions for acceptance

    The Collaborative Family Healthcare Association (CFHA) is a member-based, member-driven collaborative organization. For us, collaboration isn't just a word in our name; it defines who we are, how we interact with each other and other organizations. We believe deeply that collaboration is an essential element necessary for re-visioning healthcare, specifically, and society, generally. CFHA promotes a comprehensive and cost-effective model of healthcare delivery that integrates mind and body, individual and family, patients, providers and communities. CFHA achieves this mission through education, training, partnering, consultation, research and advocacy.

     

    The CFHA Annual Conference focuses on the many and varied aspects of collaborative and integrated healthcare. It has become the premiere conference of its type, bringing together practitioners, researchers, students, academicians, corporations, governmental agencies, foundations and many others. More than 400 attendees are expected at the 2010 CFHA Conference in Louisville, Kentucky. The three-day conference includes preconference workshops; three plenary sessions and over 50 educational breakout sessions.

     

    The theme of CFHA's 2010 Conference is "Remaking National Healthcare - Collaborative Care for a Healthier America." The world is watching as the United States reforms its clinical, operational, financial, and political processes related to healthcare. This happens through large-scale reform in Washington, DC and small-scale reforms in individual states, cities, and clinics. Where is collaborative care in the context of current reform efforts? What other policy and financial reforms would make a collaborative and family-centered healthcare model more viable in real-world settings? As researchers, educators, providers and advocates of collaborative care, it is important for us to know the latest research and practice models -- to know the implications for these models on healthcare reform efforts and to know what we can do to advance collaborative practices systemically.

     

    Louisville is a gem of a city, the 16th largest in the US and host to this year's conference. The 2010 CFHA Conference room rate is $130 per night.

     

    We seek abstracts for presentations that address the Conference theme, as well as abstracts that help advance CFHA's mission to promote a comprehensive and cost-effective model of healthcare delivery that integrates mind and body, individual and family, patients, providers and communities.

     

    Presentation proposals will be considered for any aspect of collaborative care that may be of interest to Conference attendees, including research/models/best practices that are:

    • Providing a multi-disciplinary, team-based approach to care that may go beyond medical and behavioral health integration and might incorporate other disciplines such as dentistry, nutrition and pharmacy for a more whole-person approach to care.
    • Successfully establishing medical (bi-directional) care in non-medical settings, such as community mental health centers, in particular for persons with severe mental illness.
    • Succeeding in connecting individuals to a healthcare home.
    • Empowering individuals and families as owners of their care.
    • Implementing policies and addressing reimbursement challenges that create barriers to collaborative approaches to care.
    • Training health care providers to work in teams in collaborative settings.

    You may also want to consider:

    • Core knowledge - geared to basic or introductory level practitioners
    • Specialty topics - may include current ideas for responding to unique situations encountered in the field and other issues not typically addressed in core knowledge tracks; can be tailored to basic or advanced level.
    • Case review - comprehensive patient review and course of care; may be shared in a review format or presented from the patient's perspective

    Please keep in mind that Conference sessions are intended for educational purposes and are non-commercial forums. Presenters should refrain from specific product endorsement whenever possible and under no circumstances should the session be used as a place for direct promotion of a presenter's product, service or monetary self-interest.

     

    Proposals will be reviewed and selected based upon the following criteria:

    • Consistency with the conference theme and/or mission of CFHA;
    • Innovative and unique topics, or cutting-edge and timely subjects;
    • Thorough description of the presentation and learning objectives;
    • Anticipated attendee interest in the subject matter;
    • Level of participant interaction or format of the presentation;
    • Presentation experience of the educator(s).

    Terms & Conditions

     

    Should your proposal be selected for presentation at the CFHA Conference, all presenters must agree to the following:

    1. Each presentation proposal must designate a Primary Contact Person. This individual agrees to receive all correspondence and accepts responsibility for conveying and confirming Conference-related information with all presenters.

     

    2. In accordance with Essential Areas and Policies of the ACCME and the rules of the FDA and AMA, presenters are required to disclose to the activity audience any relevant financial interests or other relationships with the manufacturer(s) of commercial product(s) and/or provider(s) of commercial services discussed in an educational presentation. If your proposal is accepted for presentation, you will also be asked to disclose relevant financial relationships of your partner/spouse as well. This data will be used to ensure that your involvement in this activity is monitored for bias and/or lack of content balance.

     

    3. Presenters must register and pay to attend the CFHA Conference. Registration fees for the 2010 CFHA Conference will be comparable to 2009 registration fees: $350 for CFHA members, $395 for non-members, $200 for one-day registrations. (Fees are subject to change; more details will be provided in the coming months.)

     

    4. Presenters are responsible for their own travel and hotel arrangements and assume all related costs. Accommodations must be reserved at the designated CFHA Conference hotel.

     

    5. Presenters must confirm that Appropriate "Releases of Confidential Information" have been obtained for all client materials and copyrighted materials that will be used or recorded as part of this presentation. The responsibility for protecting client confidentiality rests with the presenters.

     

    6. Presenters will be required to provide handout materials, class outline or other related resources in advance to be incorporated into the Conference proceedings. In lieu of printed handout materials, CFHA will offer electronic options to these resource materials for Conference participants. Documents should be submitted in Word, PDF or PowerPoint format.

     

    7. PowerPoint presentations must be delivered to the CFHA Conference Manager at least 30 days prior to the Conference. The Conference AV team will pre-load your presentation onto the classroom PC computer prior to the session. We will also expect presenters to grant permission to publish presentations on the CFHA web site as a resource to Conference registrants.

     

    8. For presentations involving research with human subjects, the presenters confirm that the research has received approval from the appropriate Institutional Review Board and the data has been collected in an ethical manner.

     

    9. All presenters agree to be photographed, videotaped and/or recorded by the Collaborative Family HealthCare Association, or any supplier contracted by CFHA. CFHA or any supplier contracted by CFHA shall own all rights of any kind in perpetuity in said photography, videotaping and/or recording.
