    "Evolving Models of Collaborative Care: What They Look Like, How to Design Them, How to Evaluate Them"

    Workshop Content
    This workshop will describe the evolution of collaborative care from co-location of mental health professionals into primary care to newer models created to improve quality of mental health care and outcomes of populations of patients with a specific illness such as major depression. The workshop will cover concepts such population-based care, stepped care, the Wagner model of chronic illness care, disease registries, and how to integrate choice of evidence-based psychotherapies and medications. Key issues that will also be covered include optimal ways to identify populations with depression, training of mental health case managers, optimal composition of professionals in a mental health team, measuring success and quality of program, tracking outcomes, and funding of programs.
    Wayne Katon, MD, is Professor and Vice Chair of the Department of Psychiatry at the University of Washington Medical School, where he is also the director of an NIMH-funded Primary Care-Psychiatry Fellowship. He has received numerous teaching and research awards from the American Academy of Family Physicians, the Academy of Psychosomatic Medicine, and the American Psychiatric Association. He is Editor-in-Chief of General Hospital Psychiatry. Dr. Katon is an extraordinary researcher, having written over 400 peer-reviewed journal articles and chapters, particularly focused on the prevalence of anxiety and depressive disorders in primary care, the relationship of psychiatric disorders to medically unexplained symptoms such as headache and fatigue, and the impact of depression and anxiety on patients with chronic medical illness. In recent years, his research has focused on developing innovative models of integrating mental health professionals and other allied health personnel into primary care.

    Workshop Content
    This workshop will present a variety of approaches to teaching about collaborative team practice, from concepts in the classroom to techniques at the clinical site. The workshop will describe an overarching systemic framework for primary care; confront the differing professional cultures that must blend to be a true team, and various approaches to the inter-professional education that is key to providing a successful medical home for patients and health professionals alike. The afternoon will included didactics, video, exercises, and other participation. Come as an individual participant, or bring your team!
    Robert A. Cushman, M.D., is Professor and Chair of the Department of Family Medicine at the University of Connecticut School of Medicine and Saint Francis Hospital and Medical Center. A graduate of Amherst College and Emory University School of Medicine, he completed his Family Medicine Residency at the University of Rochester/Highland Hospital. During a fourth year there as Chief Resident, he also pursued a yearlong Family Therapy Training Program Seminar and Group Supervision. At the University of Connecticut, he served as Medical Director of the ambulatory training practice and then as Residency Program Director prior to being named Chair in 1997. His work has focused on the integrated biopsychosocial model, strategies for teaching Community-Oriented Primary Care, "translating research into practice" around asthma guidelines, and promoting collaborative practice and leadership models. His current interest is in promoting the Patient (and Family)-Centered Medical Home as an ideal model for the delivery of healthcare services.

    Susan H. McDaniel, PhD, is Professor of Psychiatry and Family Medicine, Associate Chair of the Department of Family Medicine, and Director of the Institute for the Family at the University of Rochester School of Medicine & Dentistry. Her special areas of interest are behavioral health in primary care, and family dynamics and genetic conditions. Dr. McDaniel has received many awards, most recently the Award for Distinguished Contribution to Education from the Association of Medical School Psychologists in 2004, and the American Psychological Foundation/Cummings PSYCHE Prize in 2007. Dr. McDaniel was the first psychologist to complete the Bureau of Health Professions Primary Care Policy Fellowship in 1998.
    Dr. McDaniel is the author of many journal articles, and co-editor with Thomas Campbell, M.D. of the multidisciplinary journal, Families, Systems & Health. She co-authored or co-edited 11 books, translated into 8 languages.

    "Creating the Financial Model for Behavioral Health as Part of Medical Practice" Rodger Kessler PhD, ABPP; Daniel Bruns PsyD; Brent Van Dorsten PhD

    Workshop Content
    Collaborative care, for all its advantages, must generate sufficient revenue to cover its costs if it is to remain a viable and sustainable model of care. This workshop is designed for clinicians, administrators and policy makers concerned with sustainable collaborative primary care. It will be presented by faculty who have wrestled with the successful creation of financially viable behavioral health practices in medicine, at both personal and policy levels. It will identify the elements that clinicians and clinic administrators must master if they expect to flourish in medical settings.
    The following areas will be addressed in the workshop:
    • Making the business case for collaborative care;
    • Understanding the program from clinical, financial, payor, policy maker, and Physician Hospital Organization perspectives;
    • Consultation, screening and brief intervention billing codes;
    • Relative Value Unit (RVU) based payment;
    • Barriers to reimbursement and how to overcome them;
    • Negotiating with payors;
    • Case management and the 3 tiers of medical home reimbursement;
    • Understanding Medicaid and Medicare, searching the regional Medicare carriers Local Coverage Determinations (LCD's) affecting use of codes;
    • Creating a toolkit to use when discussing these issues with non behavioral health administrators financers and policy makers;
    • Case experiences-successes, failures and processes;
    • Development of a template for a financial strategy in local settings.
    Using the template, participants will also develop a financial model for their own settings. Consultation will be provided by the faculty to refine your implementation plan.
    Rodger Kessler PhD, ABPP -- Research Assistant Professor, Department of Family Medicine, Center for Translational Science, University of Vermont College of Medicine, Berlin Family Health, Fletcher Allen Healthcare

    Daniel Bruns, PsyD, practices with Health Psychology Associates in Greeley, CO. He chairs the Clinical Health Services Committee for the American Psychological Association's Division of Health Psychology, and for the past ten years has advised the Colorado Division of Worker's Compensation in the development of their evidence-based guidelines. He has developed and published a number of psychological tests, including the BHI and the CPS. In addition, he has over twenty publications across a range of subjects, and has given numerous national presentations.

    Brent Van Dorsten, PhD, is an Associate Professor in the departments of Physical Medicine & Rehabilitation, Anesthesiology, and Psychiatry at the University of Colorado School of Medicine, where he also directs the Behavioral Medicine Fellowship program in the Department of Physical Medicine & Rehabilitation. He is on the Board of Directors of the Society of Behavioral Medicine and served for two years on the APA's Continuing Education Committee. Dr. Van Dorsten has authored twenty publications and given almost one hundred presentations to regional, national, and international audiences.
